Space is limited, we anticipate this conference to fill up early. Make sure you register soon to guarantee your space! The registration process is first come first served and the final registration deadline is May 15th, 2010. Florida FFA has switched to an online registration system. You will need to visit the Florida FFA website to register you and your students. This form must be completed for each attendee attending the conference, including advisors and chaperones. Florida FFA and the Division of Forestry requires that each student complete and submit the proper paperwork. These forms are attached to this email. There are five forms, all are attached and in a PDF format. Highlighted sections are to help bring attention to necessary information. Students are not allowed to remain at the camp without having all the forms completed. The cost of Forestry Camp is:
Payment must be sent in to the Florida FFA Association by May 1st. You spot will not be completely reserved until we receive your payment. Credit cards can be used, but will be charged a 5% processing fee. Fees are non-refundable unless cancellations are made 48 hours in advance to check-in.
Please remember that the advisor attendance policy is in effect for Forestry Camp and all students must be accompanied by a designated school district representative. In the event that the designated representative is not the FFA advisor, please submit a letter of designation from the school principal or superintendent. If your designated school district representative is another FFA advisor from your county, it is imperative that all parties are aware before arriving at the conference. Please do not automatically assume that another teacher from your county will be the chaperone for your students. All chaperone arrangements must be documented by your county supervisor, principal or superintendent.
If you, the advisor, are not attending Forestry Camp with your students, a letter must be received in the FFA Office NO LATER than July 1st-this is NOT a postmark date! Faxed copies will be accepted. Please do not show up without this letter or you will be asked to return home with your students.
- Forestry Camp is not open to 6th and 7th grade FFA members.
- 8th graders who will be in the 9th grade during the 2010-2011 school year will be permitted to attend.

This form only needs to be filled out if you intend to pick up your child before the end of camp (10:00 a.m. Friday).
If you child has medical insurance, please attach a copy of the card (front and back).
Please do not send the original card. Even if your child caries the original card, please attach a copy to the form.
This form must be notarized.
Authorization for Administration of Medication:
The upper portion of this form gives us permission to administer medication at camp, either over-the-counter or prescribed.
The lower portion is for the prescribing physician and only necessary if your child is taking any prescription medication.
